Job Description

As a Clinical Pharmacist at Pharmacy Associates, you will play a key role in medication management and patient care. You will collaborate closely with healthcare professionals to ensure the safe and effective use of medications. Responsibilities

  • Perform medication reconciliation during patient transitions of care.
  • Advocate for patient safety and effective medication use.
  • Develop protocols for safe medication practices.
  • Conduct clinical research and contribute to case studies.
  • Participate in continuing education and professional development activities.
  • Implement strategies for improving patient health outcomes.
  • Utilize electronic health records to track patient progress.
  • Collaborate with insurance providers to manage medication costs.

Requirements

Education
  • Bachelor's degree in Pharmacy
  • Postgraduate diploma in Clinical Pharmacy is preferred
Experience
  • 2+ years of experience in a clinical setting
