Job Description

We are seeking a Clinical Pharmacist with a strong background in medication management and patient consultation. The ideal candidate will play a vital role in optimizing drug therapy for our patients, ensuring safety and efficacy while collaborating closely with healthcare professionals.

Responsibilities

• Conduct comprehensive medication reviews and assessments.
• Advise on the selection of drug regimens based on patient-specific factors.
• Engage with patients to promote adherence to medication therapies.
• Collaborate with medical teams on clinical rounds.
• Participate in staff education and development programs.
• Lead quality assurance initiatives related to medication safety.
• Develop patient education materials and resources.
• Act as a resource for drug information inquiries.
• Assist in inventory management and procurement of medications.
• Maintain knowledge of the latest developments in pharmacotherapy.

Requirements

Education

• Bachelor's degree in Pharmacy
• PharmD or equivalent is preferred

Experience

• 2+ years of experience in clinical pharmacy practice

Certifications

• Registered Pharmacist with SAPC
• Clinical Pharmacy Certification (if applicable)
