Job Description

As a Care Navigator, you will be instrumental in guiding patients and their families through the often complex healthcare system. Your role will ensure that they receive adequate care and support tailored to their health needs.

Responsibilities

  • Guide patients and families in understanding their healthcare options and services.
  • Perform health assessments and interact directly with patients to evaluate their needs.
  • Collaborate with care teams to formulate effective care strategies.
  • Educate patients about treatment protocols and available resources.
  • Promote patient advocacy and support throughout their treatment journey.
  • Track patients' progress and adjust care plans as needed.
  • Engage with community services to expand patient support networks.
  • Document all patient interactions comprehensively and accurately.
  • Facilitate workshops and informational sessions for patients and families.
  • Contribute to interdisciplinary meetings to discuss patient progress and strategies.

Requirements

Education
  • Bachelor's degree in Nursing, Social Work, Psychology, or a related field
  • Master's degree is advantageous
Experience
  • Minimum of 3 years' experience in healthcare or social services
