Responsibilities

• Conduct medication reconciliation during patient admission and discharge.
• Review prescriptions for appropriateness and accuracy.
• Advocate for patient safety by reducing medication errors.
• Provide drug information to healthcare teams and patients.
• Engage in health promotion activities related to community health.
• Guide nursing staff on medication administration protocols.
• Ensure storage of pharmaceuticals complies with legislative requirements.
• Participate in pharmacy audits and performance evaluations.
• Aid in the development of pharmacy policies and protocols.

Requirements

Education

• Bachelor's degree in Pharmacy or equivalent
• Postgraduate studies in Clinical Pharmacy preferred

Experience

• 3+ years of experience in clinical pharmacy settings
