Responsibilities

• Participate in multidisciplinary team meetings to assess patient care.
• Review laboratory results and recommend changes to therapy accordingly.
• Assess and advise on potential drug interactions and contraindications.
• Conduct health screenings and educate patients on medication adherence.
• Assist with formulary management and medication procurement.
• Supervise pharmacy support staff and interns.
• Engage in continuous professional development and training.

Requirements

Education

• Doctor of Pharmacy (Pharm.D.) degree

Experience

• Experience in a clinical or inpatient pharmacy setting is preferred
