Job Description

As a Clinical Pharmacist, you will provide high-quality pharmaceutical care to patients. This includes assessing medication regimens, educating patients and healthcare staff on medication therapy, and ensuring compliance with legislation and best practices. Attention to detail and communication skills are essential in this role.

Responsibilities

  • Review patient medication regimens for safety and efficacy.
  • Coordinate with healthcare providers for medication therapy management.
  • Educate patients on chronic disease management through medication.
  • Document and monitor medication errors or adverse events.
  • Support the clinical pharmacy internship program.
  • Participate in the pharmacy's health promotion initiatives.
  • Monitor compliance with pharmacy policies and protocols.
  • Foster relationships with patients to enhance medication adherence.

Requirements

Education
  • Doctor of Pharmacy (Pharm.D.) is advantageous
Experience
  • 5+ years in a hospital or clinical pharmacy setting preferred
