Job Description

The Behavioral Case Manager will engage with individuals facing behavioral health challenges, coordinating essential services and resources that enhance their overall well-being. This role calls for a comprehensive understanding of mental health and substance abuse, paired with excellent communication and organizational skills.

Responsibilities

  • Assess behavioral health needs and develop effective care strategies.
  • Ensure seamless communication between clients and multidisciplinary teams.
  • Track and report client progress to relevant stakeholders.
  • Facilitate access to necessary community resources and services.
  • Provide emotional support and guidance to clients facing challenges.
  • Document all interactions and case developments in an organized manner.
  • Participate actively in case reviews and treatment planning sessions.
  • Educate clients about coping strategies and community support systems.
  • Review and analyze complex cases to enhance care strategies.
  • Stay updated on healthcare policies affecting mental health support.

Requirements

Education
  • Bachelor's degree in Social Work or Psychology
  • Relevant certification in mental health or substance abuse treatment preferred
Experience
  • A minimum of 3 years working directly with individuals facing behavioral health challenges.
