Responsibilities

• Perform thorough assessments to identify patients' functional limitations.
• Develop and execute treatment plans that align with patients' goals.
• Track and document patients’ progress on a regular basis.
• Facilitate communication between patients and other healthcare providers.
• Implement community programs that promote health and wellness.
• Participate in case discussions and team meetings.
• Engage in research activities to enhance service delivery.
• Utilize evidence-based practices to inform therapy techniques.
• Assist in the development of patient education materials.
• Support clients in understanding their rights and responsibilities in treatment.

Requirements

Education

• Bachelor’s degree in a relevant healthcare discipline
• Master’s degree in a healthcare-related field is advantageous

Experience

• 3+ years of experience in a similar role within the healthcare sector
