Community Health Worker

Job Description

We are seeking a passionate Community Health Worker who will be responsible for bridging healthcare services and the community. In this role, you will engage with diverse populations, providing health education, support, and connecting individuals to vital resources.

Responsibilities

• Develop and implement community outreach programs.
• Educate community members on preventive health measures.
• Cultivate relationships with local health providers and stakeholders.
• Assist clients in completing health insurance applications and enrollment.
• Provide translation services for non-English speaking clients as needed.
• Organize and facilitate community health events.
• Continuously assess the needs of the community to inform health programs.

Requirements

Education

• Bachelor's degree in Social Work, Nursing, or related field
• Master's degree is a plus.

Experience

• 3+ years of experience in health education or community outreach
