Clinical Pharmacist

Job Description

We are in search of a dynamic Clinical Pharmacist to join our team in Johannesburg. In this role, you will be responsible for ensuring the safe and effective use of medications, working closely with patients and healthcare providers to optimize therapeutic outcomes. As a critical member of the healthcare team, you will apply your clinical knowledge to contribute to our mission of enhancing patient care. Your expertise will play a vital role in medication management, patient counseling, and conducting medication reviews to ensure optimal therapy.

Responsibilities

• Conduct thorough medication reviews and assessments.
• Collaborate with multidisciplinary teams to optimize medication therapies.
• Provide drug information and education to patients and health care providers.
• Supervise medication dispensing processes and ensure compliance with regulations.
• Participate in clinical rounds and provide recommendations on medication management.
• Monitor patient outcomes and adjust therapy as needed.
• Develop and implement patient care plans.
• Provide counselling to patients on medication adherence and safety.
• Stay updated with the latest pharmacy and clinical advancements.
• Participate in quality assurance and improvement initiatives.

Requirements

Education

• Bachelor's degree in Pharmacy or Pharm.D.
• Registration with the South African Pharmacy Council is mandatory.

Experience

• 3+ years of clinical pharmacy experience preferred
