The successful candidate will be responsible for patient medication management, providing pharmaceutical care, and collaborating with healthcare professionals to ensure optimal patient health outcomes. Responsibilities

• Conduct comprehensive medication reviews for chronic disease management.
• Provide pharmaceutical care consultations to patients and healthcare teams.
• Assist in implementing clinical pathways and medication protocols.
• Support educational initiatives related to medication therapy management.
• Maintain comprehensive documentation of all clinical activities.
• Participate in continuing education and professional development.
• Contribute to cost-effective medication use and formulary management.
• Engage in patient safety initiatives to reduce medication errors.
• Advocate for patient-centered care initiatives within the organization.

Requirements

Education

• Bachelor's degree in Pharmacy or related field
• Master's degree in Clinical Pharmacy preferred

Experience

• 5+ years of clinical pharmacy experience

Certifications

• Registered Pharmacist (SA)
