We are seeking a qualified Clinical Pharmacist to join our Cape Town team. The successful candidate will be responsible for patient medication management, providing pharmaceutical care, and collaborating with healthcare professionals to ensure optimal patient health outcomes. Responsibilities

  • Conduct comprehensive medication reviews for chronic disease management.
  • Provide pharmaceutical care consultations to patients and healthcare teams.
  • Assist in implementing clinical pathways and medication protocols.
  • Support educational initiatives related to medication therapy management.
  • Maintain comprehensive documentation of all clinical activities.
  • Participate in continuing education and professional development.
  • Contribute to cost-effective medication use and formulary management.
  • Engage in patient safety initiatives to reduce medication errors.
  • Advocate for patient-centered care initiatives within the organization.

Requirements

Education
  • Bachelor's degree in Pharmacy or related field
  • Master's degree in Clinical Pharmacy preferred
Experience
  • 5+ years of clinical pharmacy experience
