Responsibilities

• Evaluate and diagnose patient health concerns.
• Work closely with multidisciplinary teams for holistic patient care.
• Prescribe treatments and medications appropriately.
• Deliver patient education concerning treatments and care plans.
• Participate in research and data collection for quality metrics.
• Advocate for patient health and rights.
• Lead health promotions and disease prevention efforts.
• Provide telehealth services as necessary.
• Maintain accurate and timely patient records.

Advantageous

• Previous experience in a primary care setting: Experience gained in a general practice or primary care environment.
• Knowledge of electronic health record (EHR) systems: Familiarity with EHR platforms will be beneficial.
