Responsibilities

• Conduct routine screenings and health assessments.
• Prescribe and manage treatment therapies.
• Facilitate referrals to specialists and other services.
• Educate and empower patients regarding lifestyle changes.
• Collaborate with clinical teams to ensure optimal patient outcomes.
• Promote health education in community settings.
• Maintain compliance with healthcare regulations.
• Provide telemedicine consultations as required.
• Evaluate and improve clinical practices within the team.

Advantageous

• Previous experience in a public health setting: Experience working in community health or public health initiatives.
• Certification in Diabetes Education: Certification to provide diabetes education and management.
