Responsibilities

  • Provide health education to patients on wellness and preventive care.
  • Support chronic disease management through ongoing patient follow-up.
  • Engage in quality improvement initiatives to enhance patient care services.
  • Facilitate referrals to specialists and other healthcare providers as necessary.

Requirements

Education
  • Master's degree in Nursing or related field
  • Registered Nurse qualifications
Experience
  • 2+ years of experience in clinical nursing
