Job Description

As a Social Worker at Community Care Solutions, you will play a vital role in supporting individuals and families dealing with challenges such as mental health issues, substance abuse, and domestic problems. Your expertise will help clients navigate their circumstances, empower them to improve their well-being, and connect them with necessary resources.

Responsibilities

  • Assess client situations and provide appropriate interventions.
  • Create individualized care plans that address both emotional and practical needs.
  • Support clients in accessing community resources and navigating bureaucratic processes.
  • Facilitate support groups and community meetings to foster peer support.
  • Document all client interactions using secure databases.
  • Engage in continuous professional development and training.
  • Collaborate with educational institutions for youth outreach programs.
  • Provide public education on social issues affecting the community.
  • Participate in regional meetings and workshops to enhance service delivery.
  • Encourage community involvement in support initiatives.

Requirements

Education
  • Bachelor's degree in Social Work or Psychology
  • Master's degree in Social Work is highly preferred
Experience
  • 3+ years of experience in direct client services or social work.
