Job Description

Responsibilities

  • Assess and diagnose health conditions of patients.
  • Create personalized care plans based on patient needs.
  • Monitor ongoing patient health and adjust treatment as necessary.
  • Educate patients on health maintenance and disease prevention.
  • Engage in interdisciplinary team meetings to discuss patient care.
  • Document all patient interactions and care plans thoroughly.
  • Conduct follow-up visits to evaluate patient progress.
  • Participate in community outreach programs to promote health.
  • Assist in training and mentoring junior staff.
  • Respond to patient inquiries and provide support.

Requirements

Education
  • Doctor of Nursing Practice (DNP) is preferred
Experience
  • Experience in primary care or specialty clinics
Technical Skills
  • Electronic Health Records (EHR)
Soft Skills
  • Problem-solving
Certifications
  • Certified Nurse Practitioner (CNP)
Languages
  • Zulu: Basic
