• Stay up-to-date with new medications and treatment protocols.
• Participate in interdisciplinary rounds and contribute to patient care discussions.
• Monitor patient progress and adjust prescriptions as necessary.
• Provide education on disease states and pharmacotherapy.
• Document clinical interventions and outcomes in patient records.
• Ensure compliance with pharmacy regulations and standards.

Requirements

Education

• Bachelor's degree in Pharmacy or PharmD
• Registered Pharmacist with SAPC

Experience

• 3+ years experience in a clinical pharmacy setting
