Responsibilities

• Evaluate patients for drug therapy effectiveness, safety, and adherence.
• Advise patients on health and wellness resources available in the community.
• Ensure pharmaceutical services align with best practices and standards.
• Document and maintain patient records accurately and confidentially.
• Coordinate with insurance providers regarding medication coverage.
• Build and maintain relationships with physicians and other pharmacy stakeholders.
• Facilitate patient access to needed medications and healthcare services.
• Lead patient education sessions on medication self-management.

Requirements

Education

• Bachelor's degree in Pharmacy or related field
• Postgraduate qualification in a specialized area of pharmacy is an advantage

Experience

• 3+ years of experience as a practicing pharmacist
