Job Description

We are seeking a skilled Registered Nurse to provide essential healthcare services to patients in their homes. You will be responsible for evaluating patient conditions, implementing care plans, and working closely with other healthcare providers to foster optimal health and wellbeing.

Responsibilities

• Evaluate patient health conditions and gather necessary health information.
• Create and adjust care plans in collaboration with multidisciplinary teams.
• Administer medical treatments and medications effectively.
• Track and record vital signs, symptoms, and reactions to treatments.
• Provide consultation and emotional support to patients and their families.
• Liaise with other healthcare professionals regarding patient care.
• Conduct educational sessions on health maintenance for patients.
• Adhere to healthcare regulations and maintain high-quality care standards.
• Uphold accurate documentation and manage patient files securely.
• Engage in improvement initiatives to enhance service delivery.

Requirements

Education

• Bachelor's degree in Nursing or related field
• Diploma in Advanced Nursing Practice preferred

Experience

• At least 2-3 years of experience in a clinical or home health setting.

Technical Skills

Certifications
