Clinical Pharmacist

Job Description

We are seeking a Clinical Pharmacist to enhance our pharmacy team. You will be responsible for reviewing patient medication regimens, providing drug information, and ensuring adherence to best practices in pharmaceutical care.

Responsibilities

• Review patient charts and medication orders for appropriateness.
• Monitor lab results and adjust medication therapies accordingly.
• Educate patients on the importance of adherence to therapy.
• Support the pharmacy team in achieving operational goals.
• Stay current with pharmaceutical advancements and best practices.
• Contribute to quality improvement initiatives within the pharmacy.

Requirements

Education

• Bachelor's degree in Pharmacy or Pharmaceutical Sciences
• Postgraduate qualification in Clinical Pharmacy is an advantage

Experience

• 3-5 years of clinical pharmacy experience

Certifications

• Certified Diabetes Educator

Languages

• Afrikaans: Conversational
• English: Fluent

• Experience with MTM Services: Experience providing medication therapy management (MTM) services to patients.
• Knowledge of Specialty Pharmaceuticals: Familiarity with specialty medications and their management.
