Clinical Pharmacist

Job Description

We are seeking a Clinical Pharmacist in Durban who is passionate about patient care and medication safety. In this role, you will assess and manage pharmaceutical care plans, working alongside healthcare teams to deliver exceptional patient outcomes.

Responsibilities

• Conduct patient consultations to assess medication therapy and manage prescriptions.
• Collaborate with healthcare professionals to optimize drug therapy outcomes.
• Review and interpret medication orders to ensure safe and effective pharmacotherapy.
• Provide drug information and education to patients and medical staff.
• Ensure compliance with regulatory standards and best practices within the pharmacy.
• Monitor patient outcomes and adjust therapy based on clinical judgment.
• Assist in managing inventory and ensuring medication availability.
• Participate in continuing education and professional development activities.
• Contribute to quality improvement initiatives within the pharmacy.
• Document medication therapy management activities in patient records.

Requirements

Education

• Bachelor of Pharmacy (BPharm) or equivalent
• Registration with the South African Pharmacy Council is essential

Experience

• At least 2 years of clinical pharmacy experience preferred
