Job Description

We are seeking a dedicated Pharmacist to join our team, who will provide medication therapy management and patient education while working collaboratively with healthcare providers. Your expertise will help us deliver exceptional pharmaceutical care that addresses the unique needs of our patients.

Responsibilities

• Evaluate health information and assess medication therapy outcomes.
• Work collaboratively with multidisciplinary teams to improve patient safety.
• Implement pharmacy initiatives to enhance the quality of care.
• Participate in quality assurance and improvement activities.
• Provide immunizations and health screenings as required.
• Facilitate communication between patients and healthcare providers regarding medications.
• Guide financial conversations regarding medication costs and alternatives.
• Ensure compliance with all regulatory guidelines and standards.

Requirements

Education

• Doctor of Pharmacy (PharmD) degree preferred

Experience

• 2-3 years of experience in a retail or clinical pharmacy setting
