Job Description

We are seeking a dedicated Clinical Pharmacist to join our team in Pretoria. In this role, you will be responsible for ensuring the safe and effective use of medications for our patients. Your expertise will play a crucial role in optimizing therapeutic outcomes and providing education to both patients and healthcare professionals.

Responsibilities

  • Evaluate patient medication regimens and optimize therapy.
  • Advise on the safe and effective use of medications within the healthcare team.
  • Develop educational materials for patients regarding their treatments.
  • Participate in quality improvement initiatives.
  • Conduct medication reconciliation at transitions of care.
  • Stay updated on new medications, policies, and guidelines.
  • Provide pharmaceutical care in accordance with best practices.
  • Collaborate with physicians to manage complex medication therapies.
  • Ensure appropriate inventory management in collaboration with the pharmacy team.

Requirements

Education
  • Bachelor's degree in Pharmacy or PharmD
  • Advanced degree in Clinical Pharmacy is advantageous
Experience
  • 3+ years of experience in clinical pharmacy practice, particularly in a hospital setting
