The ideal candidate will provide comprehensive patient care, contributing to health assessments, diagnosing conditions, and implementing treatment plans. Responsibilities

  • Evaluate and manage chronic conditions in collaboration with the healthcare team.
  • Conduct follow-up visits and modify treatment plans as necessary.
  • Provide mentorship to student nurses and new staff members.
  • Actively contribute to team meetings and patient care conferences.
  • Participate in research and quality assurance initiatives.
  • Assist in the coordination of patient referrals and follow-ups.

Requirements

Education
  • Bachelor's degree in Nursing
  • Master's degree in Advanced Practice Nursing is advantageous
Experience
  • 3+ years of experience in primary care settings
