Job Description

As a Healthcare Social Worker, you will play a crucial role in providing emotional and psychosocial support to patients and their families. Your responsibilities will include assessing patient needs, coordinating care with healthcare professionals, and facilitating access to community resources.

Responsibilities

  • Assess patients and families’ emotional and psychosocial needs efficiently.
  • Develop strong relationships with patients and provide consistent emotional support.
  • Coordinate with medical staff to ensure comprehensive care.
  • Lead support groups for family members.
  • Educate families about end-of-life issues and coping mechanisms.
  • Prepare and maintain documentation of patient care.
  • Engage in community outreach to promote hospice services.
  • Utilize problem-solving skills to address patient and family concerns.
  • Assist with advanced care planning and decision-making processes.
  • Attend training sessions to stay updated on best practices in social work.

Requirements

Education
  • Bachelor's degree in Social Work or related field
  • Master's degree in Social Work is preferred
Experience
  • 2-3 years of experience in medical social work, particularly in hospice or palliative care settings.
