Clinical Pharmacist

Job Description

We are seeking a dedicated Clinical Pharmacist to join our dynamic pharmacy team in Cape Town. The successful candidate will be responsible for providing pharmaceutical care, managing medication therapy, and ensuring optimal patient outcomes. Responsibilities

• Review medication orders for accuracy and appropriateness.
• Develop and implement patient-specific education plans.
• Participate in quality improvement initiatives related to medication use.
• Stay updated with the latest clinical guidelines and best practices.
• Support patient access to medications through formulary management.
• Act as a liaison between patients, physicians, and healthcare teams.

Requirements

Education

• Bachelor's degree in Pharmacy (B.Pharm)
• Completion of a Postgraduate Diploma in Clinical Pharmacy (preferred)

Experience

• At least 3-5 years of experience in clinical pharmacy practice

Certifications

• Registered Pharmacist with the South African Pharmacy Council
• Board Certification in Pharmacotherapy (optional)

Languages

• English: Fluent
• Xhosa: Conversational

Advantageous

• Fluency in additional South African languages: Ability to communicate in multiple South African languages is a plus.
• Previous experience in a hospital pharmacy setting: Hands-on experience in a hospital environment preferred.
