Job Description

As a Social Worker at Community Support Services, you will play a crucial role in supporting individuals and families to overcome challenges and improve their quality of life. You will provide counselling, advocacy, and assistance in accessing community resources. Your expertise will help empower clients while collaborating with various stakeholders in the community.

Responsibilities

  • Assess clients' needs through interviews and observation.
  • Create individualized care plans to address the unique challenges of each client.
  • Advocate for clients within the community to ensure access to required services.
  • Work closely with schools and healthcare providers to coordinate support efforts.
  • Document all client interactions and maintain confidentiality as per ethical guidelines.
  • Evaluate the effectiveness of interventions and modify plans accordingly.
  • Lead community outreach programs and educational workshops.
  • Support families by providing resources and referrals.
  • Report on service delivery outcomes to management.

Requirements

Education
  • Bachelor's degree in Social Work or related field
  • Master's in Social Work is advantageous
Experience
  • 5+ years of experience in direct social services or community work
