Job Description

As a Case Management Coordinator, you will oversee and coordinate all facets of case management operations within our organization. This critical role demands exceptional organizational skills and the ability to effectively collaborate with multiple stakeholders. You will assess client needs, develop comprehensive case plans, and ensure efficient service delivery that meets our high standards.

Responsibilities

  • Conduct periodic reviews and evaluations to measure the effectiveness of case management programs and recommend improvements.
  • Participate in case conferences, meetings, and training to enhance professional knowledge and skills.
  • Ensure compliance with organizational policies, procedures, and standards related to case management.
  • Provide training and mentoring to junior staff and interns in the case management team.
  • Facilitate referrals to external agencies and resources as appropriate.
  • Monitor case progress and adapt intervention strategies as needed.

Requirements

Education
  • Master's degree in Social Work, Psychology, or related field
Experience
  • Experience working with diverse populations and knowledge of community resources.
