We are seeking a devoted Social Worker to assist with community outreach and case management. The successful applicant will work directly with clients to assess their needs and connect them with appropriate services and resources.
Responsibilities
Assess and evaluate client needs through interviews and observations.
Provide crisis intervention services and support to clients in need.
Network with community resources to enhance services available to clients.
Facilitate workshops and training sessions for clients and community members.
Document all client interactions and maintain confidentiality in all communications.
Engage in community collaboration to achieve shared goals.
Offer ongoing support and follow-up services to clients as needed.
Requirements
Education
Bachelor's degree in Social Work
Master's degree in Social Work is advantageous
Experience
2+ years of experience in social work or a related field
