• Provide valuable insights during patient care team meetings.
• Involve patients in goal-setting to foster motivation and accountability.
• Engage with healthcare partners for improved patient referrals.
• Ensure confidentiality and compliance with healthcare regulations.
• Participate in continuing education workshops and attain necessary certification updates.
