Job Description

The Community Health Worker plays a vital role in connecting community members with essential health services. This position focuses on facilitating communication and engagement between various stakeholders to empower individuals and promote overall health.

Responsibilities

  • Facilitate access to health services and resources for community members.
  • Coordinate workshops and health awareness campaigns.
  • Serve as a point of contact between the community and healthcare providers.
  • Engage with diverse populations to promote inclusive health initiatives.
  • Document community feedback and report findings to management.
  • Participate in training and professional development programs.
  • Assist in grant writing and fundraising efforts.
  • Utilize social media and other platforms for community outreach.
  • Assist in developing educational materials for the community.
  • Support community members in navigating health systems and services.

Requirements

Education
  • Bachelor's degree in Community Development, Social Work, or a related field.
Experience
  • Experience in project management and community organizing is advantageous.
