Responsibilities

  • Identify and assess client needs, providing tailored support.
  • Create and execute care plans in conjunction with clients.
  • Offer individual and group therapy sessions.
  • Collaborate with multidisciplinary teams to provide comprehensive care.
  • Educate clients about available community resources.
  • Monitor client progress and adjust plans as necessary.
  • Participate in case reviews and staff meetings.
  • Promote social justice and community engagement.

Requirements

Education
  • Bachelor's degree in Social Work (BSW)
  • Master's degree in Social Work (MSW) is an advantage
Experience
  • 3+ years of experience in community service or related areas
