Job Description

As a Child, Family, and School Social Worker, your primary responsibility will be to oversee the assessment, planning, implementation, and evaluation of patient care plans in a healthcare setting. You will collaborate closely with healthcare providers, patients, and their families to ensure the highest quality of care.

Responsibilities

  • Assess patient needs and extensively review care plans.
  • Liaise with healthcare teams to promote integrated patient care.
  • Oversee compliance with healthcare laws and standards.
  • Conduct workshops and informational sessions for families.
  • Document and report on patient progress and care updates.
  • Support patients through transitions in care and life changes.
  • Engage with community resources to benefit patients and families.
  • Facilitate communication among various departments.
  • Utilize electronic medical records (EMR) to update patient information.
  • Assist in preparing documentation for legal and regulatory reviews.

Requirements

Education
  • Bachelor's degree in a related field
  • Advanced degree in Social Work or equivalent is a plus
Experience
  • 2-4 years of experience in social services, especially in case management.

Languages
  • English: Fluent
