Job Description

We are on the lookout for a Clinical Pharmacist to enhance our team in Durban. In this role, you will provide comprehensive medication reviews, collaborate with healthcare personnel, and ensure the safe and effective use of medications. Responsibilities

  • Assess patient medications and provide recommendations.
  • Liaise with medical staff regarding medication therapies.
  • Conduct patient follow-ups to monitor medication adherence.
  • Educate patients about drug interactions and side effects.
  • Ensure accurate documentation of medication therapy management.
  • Stay updated with the latest pharmaceutical advancements.
  • Engage in interdisciplinary meetings to discuss patient care.
  • Evaluate and address medication-related problems.
  • Contribute to the development of clinical protocols.
  • Assist with inventory management and medication procurement.

Requirements

Education
  • Bachelor's degree in Pharmacy
  • Clinical Pharmacy residency is preferred
Experience
  • Minimum of 3 years in clinical pharmacy practice
