Job Description

As a Clinical Pharmacy Specialist, you will play a crucial role in providing expert medication management and pharmaceutical care to patients. Your expertise will help ensure optimal therapeutic outcomes and enhance the quality of care delivered within our healthcare team.

Responsibilities

  • Assess and document patient medication therapy outcomes.
  • Collaborate on treatment plans with healthcare providers.
  • Implement medication reconciliation processes during patient transitions.
  • Review and interpret laboratory results to adjust therapy.
  • Provide pharmaceutical care education to healthcare providers.
  • Contribute to clinical research projects and studies.
  • Serve as a resource for drug information and education.
  • Facilitate medication therapy management programs.
  • Conduct quality assurance initiatives in medication management.
  • Support health literacy initiatives within the community.

Requirements

Education
  • Bachelor's degree in Pharmacy or related field
  • Registered as a Pharmacist with SAPC
Experience
  • 3-5 years of clinical pharmacy experience preferred
