Job Description

As a Social Work Case Manager, you will play a vital role in assessing client needs and implementing effective intervention strategies. Your expertise will help guide individuals and families through challenging times towards improved well-being.

Responsibilities

  • Assess client strengths, needs, and preferences to develop tailored support plans.
  • Facilitate connections between clients and community resources.
  • Monitor ongoing client needs and adjust service plans through regular reviews.
  • Provide emotional support and guidance through individual and group sessions.
  • Collaborate with healthcare providers, educators, and other professionals to support clients.
  • Advocate for systemic changes to improve client access to services.
  • Participate in case reviews and contribute to agency-wide initiatives.
  • Engage with community stakeholders to enhance service offerings.
  • Research and refer clients to appropriate services based on assessed needs.
  • Participate in training sessions to stay current with best practices in social work.

Requirements

Education
  • Bachelor's degree in Social Work or relevant field
  • Master's degree in Social Work is an advantage
Experience
  • 3-5 years of experience in case management or similar role
