Responsibilities

  • Design and deliver community support programs tailored to client needs.
  • Identify and address barriers to client engagement and success.
  • Provide ongoing support and follow-up for clients as needed.
  • Document all client interactions and maintain confidentiality.
  • Develop partnerships with local organizations to enhance service delivery.
  • Work closely with multidisciplinary teams to provide comprehensive support.
  • Assess community needs and advocate for relevant resources.
  • Provide training for volunteers or new team members.
  • Promote awareness of social issues and resources available to the community.
  • Utilize assessments to guide interventions and support services.

Requirements

Education
  • Bachelor's degree in Social Work or Psychology
  • Master's degree in Social Work is preferred
Experience
  • 5+ years of experience in social services or counseling
