Responsibilities

• Assess patient health status and medication effectiveness regularly.
• Educate patients and caregivers on proper medication usage.
• Collaborate with multidisciplinary teams to optimize patient care.
• Maintain records of patient progress and medication history.
• Stay updated with the latest pharmaceutical developments and guidelines.
• Conduct medication reviews for hospital discharge planning.
• Participate in quality improvement initiatives within the pharmacy department.
• Mentor and train pharmacy interns and staff.
• Provide consultations to healthcare providers on drug therapy.
• Contribute to the pharmacy budget planning process.

Requirements

Education

• Bachelor's degree in Pharmacy or equivalent
• Registration with the South African Pharmacy Council
• Master's degree in Clinical Pharmacy or related field is an advantage

Experience

• 3+ years of experience in clinical pharmacy practice
