Job Description

We are seeking a dedicated Pharmacist to join our pharmacy team in Durban. The ideal candidate will play a crucial role in managing medication therapies, providing pharmaceutical care, and interacting with healthcare professionals and patients to improve therapeutic outcomes. You will ensure the safe and effective use of medications and offer advice on their proper use.

Responsibilities

• Evaluate patient needs and develop medication plans.
• Provide comprehensive drug information to healthcare staff.
• Participate in clinical pharmacy services and initiatives.
• Contribute to quality improvement programs within the pharmacy.
• Manage prescription refill requests and medication renewals.
• Implement measures to reduce medication errors.
• Conduct patient follow-ups to assess medication adherence.
• Support chronic disease management through medication therapy management.
• Maintain confidentiality and integrity of patient information.
• Assist in training and supervising pharmacy personnel.

Requirements

Education

• Bachelor of Pharmacy (BPharm)
• Advanced pharmacy practice training is advantageous

Experience

• 2+ years of pharmacy experience with a focus on patient care
