Job Description

We are seeking a dedicated Community Health Worker who can effectively liaise between healthcare providers and the community. In this position, you will advocate for health services, educate the public on health issues, and provide essential support to improve overall well-being.

Responsibilities

• Manage health information sessions for community awareness.
• Identify and refer individuals to appropriate health services.
• Participate in community health fairs and outreach events.
• Advocate for health equity and access to healthcare resources within the community.
• Collaborate with other health workers and organizations to ensure comprehensive care.
• Document and track the progress of community health initiatives.

Requirements

Education

• Degree in Public Health, Social Work, or related field

Experience

• 3+ years of experience in a community outreach role
