Job Description

We are looking for an experienced Nurse Practitioner to deliver high-quality care in East London. Your responsibilities will include conducting assessments, developing treatment plans, and ensuring optimal patient outcomes through evidence-based practices and patient education.

Responsibilities

  • Perform physical exams and health assessments for patients.
  • Diagnose and treat various medical conditions.
  • Develop and implement individualized patient care plans.
  • Communicate effectively with patients and their families regarding treatment options.
  • Monitor and evaluate patient progress and adjust treatment plans as necessary.
  • Participate in patient education to promote health and wellness.
  • Collaborate with multidisciplinary teams to ensure cohesive patient care.
  • Document all patient interactions and treatment plans in the EHR.
  • Assist with administrative tasks as necessary.
  • Stay updated on best practices and current healthcare trends.

Requirements

Education
  • Master's degree in Nursing or equivalent
  • Certification in Family Practice or equivalent
Experience
  • At least 3 years of experience in a primary care setting
