Responsibilities
Conduct comprehensive assessments to determine client eligibility for government programs.
Create tailored action plans coordinating with various support services.
Facilitate access to community resources, including housing, health care, and employment programs.
Document and track client progress in case management software.
Engage in regular communication with clients to provide ongoing support.

Requirements
Education
Bachelor's degree in social work, human services, or related field
Master's degree is a plus
Experience
