Job Description

Responsibilities

  • Provide comprehensive pharmaceutical care to patients and their families.
  • Participate in multidisciplinary rounds as a member of the healthcare team.
  • Enhance pharmacy workflows through patient-centric care approaches.
  • Review and verify medication orders for accuracy and appropriateness.
  • Manage controlled substances according to legal requirements.
  • Facilitate patient access to medication through proper insurance coding.
  • Conduct training drills for emergency preparedness in pharmaceutical care.
  • Engage in community outreach programs focused on health education.
  • Implement strategies for adherence monitoring and medication reconciliation.

Requirements

Education
  • Bachelor's degree in Pharmacy or related field
  • Additional certifications in Advanced Pharmacy Practice is advantageous
Experience
  • 3+ years of clinical pharmacy experience
