Job Description

Join our team as a Treatment Coordinator, where you will coordinate care and collaborate with healthcare professionals to enhance treatment experiences for patients. This role is critical in fostering effective patient-provider relationships and ensuring compliance with health guidelines.

Responsibilities

  • Build strong relationships with patients and their families to foster trust and cooperation.
  • Participate in regular training sessions for continued professional development.
  • Evaluate the effectiveness of treatment plans and make necessary adjustments.
  • Facilitate group therapy sessions under supervision when necessary.
  • Support crisis intervention efforts and ensure patient safety.
  • Implement strategies for managing patient appointments and communications.
  • Document patient interactions and ensure timely follow-up on treatment.
  • Liaise with external agencies and support services as needed.

Requirements

Education
  • Degree or diploma in Social Work or Counseling
  • Postgraduate qualification in Mental Health is an advantage
Experience
  • Minimum 2 years of experience in mental health or substance abuse settings.
