Job Description

As a Care Coordinator, you will provide critical support in navigating the healthcare system for patients and their families. Your responsibilities include developing care plans, facilitating communication among care teams, and helping patients understand their treatment options. Responsibilities

  • Engage actively with patients to understand their unique healthcare needs.
  • Foster strong relationships with healthcare practitioners to promote patient-centered care.
  • Update case files and maintain accurate and confidential patient records.
  • Participate in training and development programs to improve care coordination skills.
  • Ensure timely follow-ups for patient appointments and treatments.
  • Guide patients through the intake process and answer queries.
  • Evaluate patient satisfaction and suggest service improvements based on feedback.
  • Support healthcare team members in delivering seamless patient care.
