Job Description

Responsibilities

  • Evaluate clients' needs through comprehensive assessments.
  • Formulate and execute intervention plans that address client-specific issues.
  • Serve as a liaison between clients and community services.
  • Document all interactions and maintain confidentiality in client files.
  • Advocate for systemic change to benefit clients and communities.
  • Provide training and education for community groups when needed.
  • Facilitate workshops on relevant social issues.
  • Work with schools and other institutions to support clients.
  • Manage referral processes to ensure clients receive timely services.
  • Continuously monitor and reassess client situations and progress.

Requirements

Education
  • Bachelor's degree in Social Work or related field
  • Master's degree in Social Work is preferred
Experience
  • 3+ years of experience with vulnerable populations
