Responsibilities

• Provide feedback to improve medication therapy management.
• Conduct medication reconciliation for patients transitioning between care settings.
• Ensure proper documentation for controlled substances.
• Conduct regular audits to ensure compliance with health regulations.

Requirements

Education

• Bachelor of Pharmacy (B.Pharm) degree
• Registered with the South African Pharmacy Council

Experience

• 1-3 years of experience in a pharmacy setting

• Experience in hospital pharmacies: Preferably worked in a hospital pharmacy setting.
