Job Description

The Clinical Pharmacist will be pivotal in enhancing patient outcomes and ensuring optimal medication use. Your responsibilities will include conducting medication reviews and educating patients on their treatments while actively participating in interdisciplinary healthcare teams.

Responsibilities

• Evaluate the appropriateness of medications in complex patient cases.
• Act as a liaison between patients and healthcare providers.
• Provide drug utilization review and medication risk assessments.
• Maintain accurate medication records and documentation.
• Support the pharmacy's initiatives for patient safety and quality.
• Participate in clinical meetings and surgery consultations.
• Lead pharmacy education initiatives for healthcare professionals.
• Conduct research and clinical trials as needed.

Requirements

Education

• Bachelor's degree in Pharmacy
• Postgraduate qualification in Clinical Pharmacy is a plus

Experience

• 5+ years of experience in clinical settings
