Responsibilities

• Create tailored support plans in collaboration with multi-disciplinary teams.
• Deliver counseling and advocacy services for at-risk populations.
• Facilitate access to social services, housing, and mental health resources.
• Engage with community members on social justice initiatives.
• Prepare reports and present at community meetings and workshops.
• Collaborate with other professionals to enhance service delivery.
• Participate in research and grant writing for program development.

Requirements

Education

• Bachelor's degree in Social Work
• Master's degree is a plus

Experience

• 3+ years of experience in social work or related fields
